Medication & Patient Safety Certificate

The Medication & Patient Safety Certificate demonstrates your proficiency in preventing medication errors and improving patient outcomes. After earning this certificate, you will show your expertise in:

  • Identifying root causes and contributing factors to medication errors, and applying RCA and FMEA to investigate incidents and proactively mitigate risk.
  • Complying with medication error reporting standards (ISMP MERP, FDA MedWatch, The Joint Commission Sentinel Event policies) and documenting incidents accurately and ethically.
  • Applying human factors and systems thinking to reduce risk, addressing cognitive biases, workload, and environmental distractions through standardization, checklists, and workflow design.
  • Leveraging pharmacy technology (eRx/CPOE, CDSS, BCMA, ADCs, smart pumps) effectively while mitigating risks such as alert fatigue and workarounds; using scanning and verification best practices.
  • Using structured communication (SBAR, read-back, medication reconciliation) to strengthen interprofessional collaboration and reduce errors, especially during transitions of care.
  • Fostering a Just Culture and exercising safety leadership to build a proactive, non-punitive reporting environment and lead continuous improvement initiatives.

Eligibility Requirements

Candidates must be an active CPhT (through either NHA or PTCB) or a Registered Pharmacy Technician registered with their State Board of Pharmacy and meet one of the following eligibility requirements to sit for this exam: 

      Option 1. Completion of a BPTS-Recognized Training Program for Medication Safety, OR

      Option 2. A minimum of 1 year or 1,000 hours of supervised pharmacy experience related to Medication Safety.

Exam Fee

The exam fee for the Medication & Patient Safety Certificate is $49.
This fee includes one complimentary retake, if required.

Exam Details

The Medication & Patient Safety Certificate Exam is a computer-based exam comprised of 60 multiple choice questions.
Candidates will have 90 minutes to complete the exam and survey.

The exam can be taken at your preferred location, such as at home, and is available online, 24/7.

As a part of the registration process, you will have the opportunity to indicate your need for ADA exam accommodations.
After registration is completed, you will receive an email with instructions on how to complete and submit your request for accommodations.

The exam will be available beginning April 1, 2026

Instructions will be sent to candidates after applications are reviewed and approved.

The exam will be available beginning April 8, 2026

Earning Your Certificate & Badge

After passing the exam, you will receive a digital badge and certificate through Accredible. This will allow you to print a copy of your certificate and display your digital badge on your resume, your social accounts (LinkedIn, Facebook, Twitter) or even as part of your email signature.

The Medication & Patient Safety Certificate does not require renewal.
